Well-Child Pediatric Visits and Care Coordination

The intent of this strategy is to ensure that all children have access to high-quality preventive and continuous health care, including physical, mental, oral and nutritional health. It enhances well-child visits and pediatric care by embedding a developmental specialist within the practice who provides families of young children with support to promote children’s overall health and wellness, positive parenting guidance and navigation of the health care and social services systems.

Coconino

Beginning in State Fiscal Year 2019 (SFY19), the regional council has been implementing the Children’s Health Systems Change Strategy with the objective of creating two reports with information to shape and inform the work of the regional council in the areas of well-child and family care and the system of care for children’s behavioral and mental health needs.

The first report has provided the information needed to shape the expansion of the Well-Child Family Care Strategy to the northern communities of the Coconino Region. Looking to SFY21, the regional council will utilize the report in structuring an RFGA that will better ensure the strategy will be impactful and relevant to the children and families in those communities.
